Job Summary
Develop service plans, treatment plans, and individual crisis plans based on client strengths and needs. Assist consumers in gaining access to public and private resources for medical, vocational, educational, and employment services. Advocate for clients by negotiating complex systems such as special education and social services while maintaining access to housing, food, and medical care. Provide 24-hour, 7-day crisis intervention on a rotational basis and communicate significant health changes to the treatment team. Maintain case records, complete assessments, and prepare incident reports within required timeframes. This role supports individuals with intellectual and developmental disabilities and those facing mental health and substance use challenges.
Required Qualifications
- Bachelor's degree in human services related field - sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education
- Associates Degree with 12 College Credits relevant to position (Human Services, Psychology, Sociology)
- Two (2) years' experiences in the mental health/human service field
- Valid driver's license
