Blended Case Manager

Job Summary
Conduct continuous case management for adults with developmental disabilities, behavioral health issues, and substance abuse needs in compliance with contract and agency regulations. Develop and refine individual rehabilitation plans, complete functional assessments, and maintain regular supports to ensure continuity of care. Resolve crisis situations by identifying key resources for direct support and immediate intervention while serving as an advocate for clients to assume proactive roles. Maintain a close liaison with local community providers to facilitate cooperation and secure alternative resources. Monitor client Representative Payee Accounts as needed. Complete required documentation and meet productivity expectations. Requires valid driver's license, local travel availability, and scheduled emergency on-call service with a beeper.
Required Qualifications
- B.A/B.S. degree in Human Services/Social Science, Healthcare or a related field
- 3 years of MH direct care experience
- Very good communication skills
- Detail oriented
- Ability to work independently and as part of a focused team
- Knowledgeable of computers, Microsoft Outlook, Word and the Internet
- Ability to lift, carry, fold, kneel, reach, stack, stoop and bend
- Valid Driver's license
- Manual dexterity and fine motor skills
- Ability to meet established direct care service expectations including documentation of the services
- Must be available for local travel
- Must be available for scheduled emergency on-call service
- Must have the ability to meet productivity
