Job Summary
Blended Case Manager provides services to children and adolescents’ physical and behavioral health, identifies strengths, and accesses resources to enhance quality of life. Advocates for school placements and is knowledgeable about the Special Education system and Individual Placement plans. Collaborates with other service deliveries to ensure needs are met; maintains a monthly productivity goal and participates in on-call rotation. Required Qualifications
- Bachelor's degree with major coursework in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education, or
- Registered nurse or
- A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences and two years experience in direct contact with mental health consumers.
