Blended Case Manager

Job Summary
Manage a caseload of 30 consumers by meeting them in homes, the community, or the office to assess needs, develop service plans, and implement goals based on strengths. Link consumers to specified services, monitor delivery, resolve problems, and advocate when necessary while building informal support networks with family consent. Complete all required documentation, adhere to federal and state policies, and maintain awareness of fiscal requirements. Provide 24/7 crisis prevention and intervention as the primary contact, coordinate with treatment teams, and communicate barriers to supervisors. Produce minimum monthly productivity units and attend bi-weekly group supervision and scheduled face-to-face sessions with the Director.
Required Qualifications
- Bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, criminology, theology, nursing, counseling, or education or a registered nurse
- High school diploma and five years of related mental health direct care experience in social work, case management, nursing, and childcare
- Act 33 & 34 Clearances
- FBI Clearance
- Act 31 Mandated Reporter Training
- Computer skills
- Command of the English language to speak, understand, write
- Good public speaking skills for small and large groups
- Solid organizational skills and time-management skills
- The ability to tolerate a stress level that may be moderate to high in dealing with demands of work requirements and crisis situations with consumers
- Ability to think clearly during times of crisis and define safety plans with staff and/or consumers
- Must demonstrate emotional intelligence when dealing with conflict between staff and/or consumers
- Must be able to negotiate, navigate, and coordinate the multiple services involved in Referrals and consumer cases
- Must have and maintain a valid driver's license and vehicle to attend consumer/agency appointments outside the office
- Must maintain automotive insurance coverage limits as per the Community Guidance Center Policy, and a copy of coverage verification must be provided to the Community Guidance Center upon each policy renewal
- Must be able to use a fax, cell phone, keyboard, automobile, iPad
