Job Summary
Bilingual Spanish Sr. Community Health Worker in Riverside, CA area responsible for enhancing frail seniors' ability to age in place and navigate the health care system. Provides person-centered care telephonically and in-person, conducts comprehensive assessments for Medi-Cal/Medicare dual-eligible members, supports care coordination, coaching, and referrals to community resources. Implements personalized care plans, participates in interdisciplinary planning, and engages with members and their families to manage chronic conditions. Requires 50%+ travel in assigned area; hybrid work mode. Education includes High School Diploma; Community Health Worker Certificate strongly preferred; must be bilingual in English/Spanish. Requires 5+ years in managed care or case management, 2+ years working with seniors, and valid driver’s license. Must adhere to confidentiality, NCQA CMS/DHCS regulations, and possess strong problem-solving and communication skills. Salary base range $25.38–$35.01/hr (annualized to $52,790–$72,821). Required Qualifications
- High School Diploma required
- Community Health Worker Certificate strongly preferred
- BILINGUAL English/Spanish
- Travel 50+% of the time
- Must have valid driver’s license, automobile insurance and reliable transportation
- 5+ years managed care, healthcare environment, lived experience or case management
- 2+ years working with seniors, conducting home visits, and working remotely strongly preferred
- Strong communication and organizational skills
- Knowledge of NCQA standards, CMS and DHCS regulations
