Job Summary
Conduct outbound and inbound telephonic outreach to members, prescribers, and pharmacy providers to deliver Medication Therapy Management (MTM) services and support the Star adherence program. Perform comprehensive medication interviews, document medications, and facilitate communication regarding client outcomes while maintaining strict HIPAA and CMS compliance. Execute clinical interventions for commercial and Medicare members through video or phone channels as part of the Personalized Member Transition and Academic Detailing initiatives. Fluency in Spanish is required for effective patient interaction.
Required Qualifications
- High school diploma or GED
- Previous experience as a Pharmacy Technician
- Valid Pharmacy technician license/certification as required by state laws
Desired Qualifications
- Active national pharmacy technician certification through either PTCB or NHA
- Fluency in both English and Spanish
- Experience as a member of a multi-disciplinary team
- Knowledge of applicable data privacy practices and laws
- Knowledge of healthcare industry practices and HIPAA regulations
