Bilingual Spanish Licensed Practical Nurse

Conduct remote patient assessments using telehealth technology to determine acuity and clinical needs while monitoring vital signs and health data via RPM devices. Triage patient concerns based on clinical guidelines, providing initial advice, direction, and escalation when necessary. Manage chronic conditions such as diabetes and hypertension by reviewing patient data, assessing trends, and collaborating with physicians and nurse practitioners to ensure comprehensive care delivery. Provide education to patients on managing chronic conditions and lifestyle changes, with a focus on serving both English and Spanish-speaking populations. Requires a current New York LPN license, two years of clinical nursing experience, and bilingual English and Spanish proficiency.
