Bilingual Spanish Intensive Care Coordinator - Health Home Plus

Monitor client progress through face-to-face, telephonic, and electronic outreach in accordance with Health Home and departmental guidelines. Complete comprehensive functional assessments to identify medical, behavioral health, and social needs, then develop and update person-centered care plans with clients and network partners. Maintain accurate caseloads by managing discharge or transfer activities and promptly address crisis situations with supervisory staff. Facilitate referrals to medical, behavioral health, and social assistance entities while assisting with entitlements, housing, and transportation. Utilize Electronic Health/Medical Record systems and NADAP database tools to document all treatment records and contact notes. Bilingual Spanish speaker required; salary $60,000/year.
