Job Summary
Conduct 6-8 daily home visits and telehealth sessions with patients, caregivers, and families to develop wellness coaching relationships focused on reducing high-risk behaviors and improving quality of life. Collaborate with advanced practice clinicians to create action plans, deliver health education materials, and assist clients in accessing community resources to address social determinants of health. Maintain patient confidentiality in accordance with HIPAA regulations while fostering trust through effective communication and problem-solving across diverse populations. Work independently within the Care Anywhere team to support the mission of empowering seniors to age well.
Required Qualifications
- Minimum (1) year of experience as a wellness or health coach or educator
- High School Diploma or GED
- Associate's degree or two years additional experience in lieu of education
- Ability to communicate positively, professionally and effectively with others

- Solid knowledge of specific diseases, lifestyle-related topics, nutrition, and exercise
- Knowledge of and experience with applying the stages of change or a similar behavior change model
- Excellent interpersonal skills with the ability to foster trust and respect among customers and coworkers
- Able to work independently and remotely
- Able to work with protected health information and maintain absolute confidentiality and privacy in accordance with HIPAA regulations and AHP policies
- Proficient computer skills including Microsoft Office Excel, Word and PowerPoint
- Ability to work a flexible schedule
- Effective written and oral communication skills

- Ability to perform mathematical calculations and calculate simple statistics correctly
