Job Summary
Conduct in-home and telephonic assessments, care planning, and client monitoring for frail, economically disadvantaged older adults and caregivers to prevent premature institutionalization. Coordinate linkages with community-based services, maintain legal and clinical documentation including service authorizations and progress notes, and implement comprehensive care plans with measurable timeframes. Ensure continuity of care through ongoing communication with clients, families, service providers, and health professionals while providing informal training to fellow staff. Participate in Case Conferences, Quality Assurance chart reviews, and outreach activities, keeping current on Medicare, Medi-Cal policies, and Federal legislation. Travel 50%+ of the time throughout Los Angeles County to visit client homes, requiring a valid driver's license and reliable transportation.
Required Qualifications
- Bachelor's Degree in Social Work, Psychology, Gerontology or related field
- BILINGUAL- Must be bilingual in English/Spanish
- 1-2 years experience working with the elderly
- 1+ year experience working in a hospital, home health agency or related setting in which the primary targeted population is older adults
- Ability to travel within program's catchment area and visits to client homes
- Ability to perform home assessments, telephone monitoring, and travel to client homes throughout Los Angeles County
- Knowledge of environmental concerns of older adults
- Theoretical understanding of the developmental needs for this population and means for intervention
- Strong organizational, verbal and written communication skills
- Demonstrated customer focus
- Ability to work well with others
- Valid driver's license

- Reliable transportation
- Must be available for Tuberculosis screening upon hire or proof of a negative screening within the last year
- Must be available for a background check
Desired Qualifications
