Job Summary
Conduct field-based support for individuals with medical and behavioral health conditions by performing assessments, developing care plans, and coordinating services with primary and specialty providers. Facilitate connections to community-based resources and entitlements while educating clients on diagnoses, medications, and treatments. Maintain bilingual Spanish communication skills to deliver these services effectively. This role is part of NADAP's Health Homes Care Management team, which assists disadvantaged populations in New York City and Nassau County through health insurance enrollment and job preparation services.
Required Qualifications
- Bachelor's Degree in human services, social work, or related field
- Associate's degree with 1 year of case management experience
- High School Diploma with 2 years of case management experience
- Excellent organizational, communication, and computer skills
- Bilingual Spanish speaker
