Job Summary
Bilingual (Spanish) Care Coordinator responsible for in-person, video, and telephonic outreach and care coordination as part of the Pathways to Success program. Provides comprehensive Integrated Assessment and Treatment Plan reviews (IM+CANS) for community-based youth and families, concentrates on family-driven, strengths-based care, maintains a caseload of youth under 21 with severe emotional disturbance, links clients to long-term services, coordinates with MCOs and partner agencies, conducts Child and Family Team (CFT) meetings, and supports transitions from hospital to community settings. Responsibilities include coordinating and delivering care across home and community settings, ensuring culturally competent stabilization and support, and collaborating with multidisciplinary teams to develop and implement treatment plans, discharge planning, and ongoing progress monitoring. Work environment includes a mix of office and community/community-based settings with substantial field activity; required to work a flexible schedule including some evenings. Minimum qualifications emphasize a bachelor’s or master’s degree in relevant fields and at least two years of clinical care coordination experience with children and adolescents, plus a valid driver’s license and reliable transportation.
Required Qualifications
- Bachelor's or Master's Degree from an accredited College or University in Social Work, Psychology, Counseling or closely related discipline
- Minimum of 2 years experience providing clinical treatment or care coordination with children and adolescents
- Valid driver’s license, proof of insurance and reliable transportation
