Job Summary
Provide individualized social work services and ongoing case coordination to adults in community residential settings. Conduct assessments, develop service plans, and connect participants with resources to support stability and wellness. Maintain accurate documentation and reporting while participating in team meetings, supervision, and interdisciplinary collaboration. This 35-hour role supports a multidisciplinary team environment focused on building trusting relationships and promoting independent living. MSW required with experience working with adult populations.
