Job Summary
Social Worker II provides social work services to residents of senior housing, delivering care coordination, counseling, crisis intervention, discharge planning, and group work to support residents in maintaining safe, independent lifestyles. Requires a Master’s degree (MSW preferred or MA in human services with two years of social work experience) and bilingual Spanish-English communication. Responsibilities include assisting residents with medical/mental health services, entitlements, home care, budgeting and income maintenance, intake and discharge processes, maintaining case records, safeguarding confidentiality, and collaborating with staff and external providers. Also involves advocacy, participant engagement in social activities, and ensuring compliance with diversity, equity and inclusion standards. Fluency in English is required; experience with older adults and mental illness/substance use is preferred.
