Bilingual Registered Nurse (RN) - Case Management

Supervise and support direct care staff to ensure compliance with policies and regulations while managing a caseload of clients and developing personalized treatment plans. Conduct comprehensive assessments and supervisory visits, documenting care plans in line with agency standards, and act as a liaison among staff, community, and healthcare providers to ensure seamless communication. Lead orientation, training, and competency verification for new hires, and provide in-field supervision and real-time guidance to clinicians. Participate in quality assurance, performance improvement, and staff development initiatives, and support on-call operations to ensure timely follow-up and accurate documentation. This full-time, daytime role includes regular local travel to patients' homes for field days and office days.
