Job Summary
Visit partner adult day care centers throughout New Jersey to build relationships with participants, families, and staff. Educate individuals about available behavioral health services and help connect patients with psychiatric and therapy resources. Serve as a liaison between patients, families, providers, and center staff while providing Spanish-English interpretation when needed. Maintain accurate documentation and uphold HIPAA standards. Work Monday through Friday between 8:30 AM and 2:30 PM, up to 30 hours per week. This role supports seniors and their families in accessing compassionate behavioral health services across Northern and Central New Jersey.
Required Qualifications
- Fluent in English and Spanish
- Excellent active listening skills, with the ability to build relationships and work effectively within different adult day center environments, policies, and procedures.
- Comfortable speaking with individuals and groups
- Valid driver's license and reliable transportation
- Willingness to travel throughout New Jersey
- Organized, dependable, and self-motivated
