Job Summary
Provide urgent health care via telephone and tele-video to patients receiving home and community-based services through state Medicaid programs. Develop and implement clinical plans of care for adults with chronic and complex conditions, including co-morbid medical and mental health diagnoses. Gathers history and physical exams, conducts diagnostics, and educates patients and families on medication usage, diet, and crisis prevention. Collaborate with interdisciplinary teams to coordinate optimal care resources while maintaining accurate medical documentation consistent with state regulations. Prescribes medication as permitted by state authority and participates in required continuing education. Requires an active NP license in Texas, Kansas, or Iowa, bilingual skills, and an MS in Nursing.
Required Qualifications
- Active, unrestricted Nurse Practitioner license in Texas, Kansas, or Iowa
- Active, unrestricted RN Compact license
- MS in Nursing
- Active national NP certification
- Bilingual or multi-language skills
- 2 years+ of experience in managing complex care cases
- Experience working with Electronic Medical Records (EMR)
- Valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in either one of the following states: Texas, Kansas, or Iowa
- Must be available for required in-person training sessions
- Must reside within a reasonable commuting distance from the posting location(s) unless an accommodation is granted
- Must be vaccinated against COVID-19 and Influenza

- Possession of DEA registration or eligibility
- Experience in managing complex care cases for developmental disabilities and chronically ill patients
