Job Summary
Provide support to patients and health care providers by offering information on Patient Support services, specific diseases, and therapies based on manufacturer materials. Assist patients in conducting assessments, act as a liaison for medication access, and address general inquiries regarding program services. Document patient information and interactions within the electronic patient record, conduct ongoing file reviews, and follow case-management protocols including data collection, follow-up, and adverse event reporting. Collaborate with reimbursement specialists and physicians to secure funding, coordinate treatment logistics with providers and clinics, and refer patients for clinical questions beyond program materials. Participate in specialized training sessions, attend pharmaceutical and patient meetings, and assist in training new Nurse Case Managers. May be required to be on standby or on call as part of this role. Full-time position.
Required Qualifications
- A Diploma in health care or a related field
- Bachelor's degree
- Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment
- Minimum of two (2) to five (5) years in the healthcare, pharmaceutical, insurance or related fields
- Fluency in French and/or English
