Bilingual Mental Health Counselor-MDFT OUD

Coordinate intensive in-home clinical care for adolescents with substance abuse and delinquent behaviors, providing group and individual counseling with supervision. Establish referral agreements with community providers, develop case management plans, and administer standardized screening tools to improve treatment completion and life functioning. Maintain accurate, compliant documentation for client medical records and attend team meetings to review treatment plans. Transport clients to appointments as needed and provide translation services in Spanish and English. Support safety planning and crisis intervention with appropriate supervision, including after-hours on-call responsibilities.
