Job Summary
Coordinate urgent, emergent, and preventive medical, dental, and mental health services for children in care, including transporting them to appointments and accompanying them during visits. Manage the full medical documentation lifecycle by collecting forms, preparing treatment authorization requests, scanning records into portals, and maintaining secure physical health files. Collaborate with cross-functional teams and healthcare providers to schedule appointments, obtain medical clearances, and ensure continuous quality improvement measures are met. Provide in-service training on medication administration, CPR, and first aid to staff and foster parents while maintaining stocked medicine cabinets and first aid kits. Report medical emergencies via the SIR/CLE process and assist program leadership during auditing. Maintain Red Cross CPR/First Aid certifications and support foster parents with medical policy education and authorization documents.
Required Qualifications
- Licensed Practical or Vocational Nurse (LPN/LVN) or higher health-related qualification (e.g., Licensed Registered Nurse (RN) with a bachelor's degree) OR Bachelor's degree in a relevant field.
- Bilingual fluency in English and Spanish
- Minimum of 1 year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care.
- 1 year of experience working in the child welfare field working with children and/or adolescents in a social service setting
- Valid driver's license, excellent driving record, a safe and clean personal vehicle, and current automobile insurance
- Ability to maintain compliance with attending agency required training programs
- Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs
- Must be open to and willing to work non-traditional hours and be available for on-call phone consultations as needed
- Maintain Red Cross CPR/First Aid Trainer's Certifications
