Bilingual Lead Care Manager

Job Summary
Engage in outreach activities to enroll new clients, complete comprehensive assessments, and develop individualized care plans for enrolled members. Coordinate services by organizing care activities, managing referrals, and maintaining regular contact with outside providers to ensure resource access. Provide education and support needs identification for youth and caregivers through in-person visits and telehealth. Utilize crisis communication and de-escalation techniques, including intervening with behavioral programs as needed, while building relationships with community providers. Work Monday through Friday with flexibility for evening shifts, requiring a valid driver's license for travel up to 90 miles.
Required Qualifications
- Bachelor's degree
- Associate degree or equivalent + 1 year of applicable work experience
- High school diploma/GED + 2 years of applicable work experience
- Applicable experience includes paid or unpaid work with youth, families, or in the social services field
- Bilingual in Spanish/English, pass bilingual proficiency assessment
- Flexible schedule with ability to work evenings as needed
- At least 21 years of age
- Driving within the community to meet with clients, attend meetings, or perform job duties at multiple locations within a specific region
- TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
- Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies' insurance policy
