Job Summary
Conduct comprehensive assessments and develop care plans for youth and families enrolled in Enhanced Care Management. Engage in outreach to enroll new clients, build rapport with enrolled members, and organize care activities through regular contact with outside providers. Manage referrals, coordinate services with county substance use disorder and specialty mental health providers, and provide education and support needs identification. Deliver services in person or via telehealth while utilizing crisis communication and de-escalation techniques. Identify community supports and resources through ongoing community outreach. Requires bilingual Spanish/English proficiency, valid driver's license, and ability to travel up to 90 miles. Full-time Monday through Friday schedule with hybrid flexibility.
Required Qualifications
- Bachelor's degree
- Associate degree or equivalent + 1 year of applicable work experience
- High school diploma/GED + 2 years of applicable work experience
- Applicable experience includes paid or unpaid work with youth, families, or in the social services field
- Bilingual in Spanish/English
- At least 21 years of age
- Driving within the community to meet with clients, attend meetings, or perform job duties at multiple locations within a specific region
- TB test clearance
- Fingerprinting clearance
- Any other state/federal licensing or certification requirements
- Valid driver's license
- Clean driving record
- Insurability through Seneca Family of Agencies' insurance policy
- Own vehicle
- Current automobile insurance

- Prior case management and community-based experience
- Lived or family experience in navigating medical and mental health, probation, child welfare, and/or education systems and resources
- Commitment to advancing racial equity
- Strong written and oral communication skills
- Strong organizational skills as demonstrated by the ability to effectively manage multiple tasks
