Job Summary
Conduct outreach to enroll new clients in Enhanced Care Management (ECM) and build rapport with enrolled members. Complete comprehensive assessments, develop individual care plans, and organize care activities by maintaining regular contact with outside providers, including county substance use disorder and specialty mental health services. Manage referrals, coordinate follow-up for needed services, and provide education to youth and caregivers through in-person or telehealth visits. Utilize crisis communication and de-escalation techniques, which may involve physical management of clients through behavioral programs as per training. Engage in community outreach to identify local supports and resources. This role requires driving up to 90 miles within a specific region and involves working Monday through Friday from 9:00 AM to 5:30 PM with flexibility. Seneca Family of Agencies serves traditionally marginalized communities with mental health and educational services, emphasizing diversity, equity, and inclusion.
Required Qualifications
- Bachelor's degree
- Associate degree or equivalent + 1 year of applicable work experience
- High school diploma/GED + 2 years of applicable work experience
- Applicable experience includes paid or unpaid work with youth, families, or in the social services field
- Bilingual in Spanish/English
- At least 21 years of age
- Driving within the community to meet with clients, attend meetings, or perform job duties at multiple locations within a specific region
- TB test clearance
- Fingerprinting clearance
- Any other state/federal licensing or certification requirements
- Valid driver's license
- Clean driving record
- Insurability through Seneca Family of Agencies' insurance policy
- Own vehicle
- Current automobile insurance
- Travel included - up to 90 miles
