Job Summary
Assess new clients for MSSP program appropriateness, certify nursing home eligibility, and conduct medical, physical, and psychological reassessments. Develop and implement written care plans approved by supervisors, provide ongoing health education to non-clinical staff and clients, and present complex cases at weekly team conferences. Collaborate with physicians, home health providers, and community services to ensure safe living environments below skilled nursing facility costs, while maintaining accurate monthly documentation and service authorizations. Travel 50%+ of the time within Los Angeles County to perform home assessments and client visits. Available Monday-Friday, 8am to 5pm with extended hours as needed. Bilingual in English and Korean with a current California RN license and 3+ years of health assessment experience required.
Required Qualifications
- Nursing degree
- Bachelor's degree in Nursing (BSN)
- Registered Nurse
- Current and active California RN license in good standing
- 3+ years experience with health assessments, level of care criteria, care planning, and case documentation
- Ability to travel within program's catchment area and visits to client homes
- Knowledge of environmental concerns of older adults
- Theoretical understanding of the developmental needs for this population and means for intervention
- Ability to perform home assessments, telephone monitoring, and travel to client homes throughout Los Angeles County
- Ability to work as a part of a team, while at the same time performing job independently
- Strong organizational, verbal, and written communication skills
- Ability to work well with others
- Demonstrates keen attention to detail and customer focus
- Ability to prioritize multiple and competing tasks
- Available to work Monday-Friday, 8am to 5pm with extended work hours, as needed and extensive simultaneous computer and telephone usage
- Bilingual in English/Korean
- Valid driver's license

- Demonstrated customer focus and experience working in a case management setting
- Demonstrated knowledge of case management techniques
- Ability to provide proof of Tuberculosis screening upon hire or proof of a negative screening within the last year
