Job Summary
Manage referrals and coordinate access to treatment services for individuals receiving substance use and co-occurring mental health services. Answer incoming calls, complete client intakes, and provide resources, education, and ongoing support to clients and their families. Schedule appointments, transportation, and other services to support engagement in treatment, while assisting with continuity of care by coordinating with family members and caregivers. Follow up on medical services identified in the client's treatment plan and accurately document all treatment coordination activities in accordance with agency policies and regulatory standards.
Required Qualifications
- Fluency in Karen and English
- Passion for improving access to culturally responsive behavioral health services
- Skill in identifying and assessing a wide range of client needs
- Knowledge of local community resources and how to use those resources for the benefit of the client
- Successful completion of 15 hours of education or training on substance use disorder, co-occurring conditions, and care coordination for individuals with substance use disorder or co-occurring conditions consistent with national evidence-based standards
- Bachelor's degree in one of the behavioral sciences or related fields
- High school diploma or equivalent
- Qualifications as a mental health practitioner or alcohol and drug counselor
- Proof of authorization to work in the United States
- Passing a background check related to the job requirements
Desired Qualifications
- At least 1,000 hours of supervised experience working with individuals with substance use disorder or co-occurring conditions
