Job Summary
Manage referrals and coordinate access to treatment services, answer incoming calls, and complete client intakes. Provide resources, education, and ongoing support to clients and families while scheduling appointments, transportation, and other engagement services. Assist with continuity of care by coordinating with family members and caregivers, following up on medical services, and facilitating referrals to substance use disorder treatment, mental health services, and primary care. Accurately document all treatment coordination activities in accordance with agency policies. This full-time role supports individuals receiving substance use and co-occurring mental health services at Wilder Recovery Services in St. Paul.
Required Qualifications
- Fluency in Karen and English
- Passion for improving access to culturally responsive behavioral health services
- Skill in identifying and assessing a wide range of client needs
- Knowledge of local community resources and how to use those resources for the benefit of the client
- Successful completion of 15 hours of education or training on substance use disorder, co-occurring conditions, and care coordination for individuals with substance use disorder or co-occurring conditions consistent with national evidence-based standards
- Bachelor's degree in one of the behavioral sciences or related fields
- High school diploma or equivalent
- Qualifications as a mental health practitioner or alcohol and drug counselor
Desired Qualifications
- At least 1,000 hours of supervised experience working with individuals with substance use disorder or co-occurring conditions
