Job Summary
Conduct assessments of consumers via telephone, video conferencing, and home visits to evaluate history, living situations, and support systems for program eligibility. Initiate immediate intervention services when required and develop, authorize, and implement initial care plans within budget guidelines. Coordinate community-based services and public benefits access while providing information and referrals to ineligible applicants. Manage crisis case management prevention and intervention, documenting all consumer information in case records and databases. Participate in interdisciplinary case management, staff orientation, and supervisions as assigned. Requires fluent English and Haitian Creole or Portuguese, a Bachelor's degree in social work or related field, and private transportation. Full-time, non-exempt position with hybrid schedule, competitive salary, and comprehensive benefits including medical, dental, vision, and retirement plans.

- Ability to lift, push, pull and carry objects weighing up to 25 pounds
- Ability to climb up and down stairs unassisted

- Work requires regular standing, stooping, and bending
Desired Qualifications
- A Bachelors' degree preferred from an accredited school in Social Work, Psychology, Counseling, Human Development, Nursing, Sociology, Criminal Justice, Public Health, Public Policy, or Gerontology plus at least two years of experience in counseling, casework or case management
- Excellent interpersonal skills with demonstrated ability to communicate (written and verbal) across all levels, including staff, family and external partners
- Strong interviewing, assessment and organizational skills
- Ability to maintain confidentiality while working with sensitive information or in sensitive situations
- Highly resourceful team player, with the ability to work effectively independently or as a team
- Demonstrates a willingness to take on new tasks with a general attitude that no task is too small, impossible, or cannot be improved
- Proficient in the following platforms: Microsoft Office (Word, Outlook, Teams, Excel, and PowerPoint), Adobe Pro, Zoom and other web-based applications
