Bilingual Home Visiting Provider

Monitor the Am I Eligible (AIE) platform, email, fax, and Zoho CRM to identify and capture incoming referrals. Review all submissions for completeness and accuracy, then enter and process them into the AIE system or designated databases. Assign completed referrals to the appropriate Home Visiting Lead and escalate urgent cases to leadership. Conduct outreach calls to non-engaged families, previously closed referrals, and those requiring additional follow-up. Assist with data reporting, maintain tracking spreadsheets, and support continuous quality improvement initiatives. Connect ineligible families with community resources and the MECA Community Health Worker Program.
