Job Summary
Coordinate and authorize long-term care and social support services for SCO enrollees, including ADL/IADL assistance, housing, meals, and transportation. Conduct initial and ongoing assessments of health and functional status to develop community-based care plans. Monitor service provision and functional outcomes while tracking enrollee transfers and adjusting service plans as needed. Provide care management via telephone, prepare documentation for enrollee files and agency databases, and report suspected fraud, waste, and abuse. Respond promptly to service requests and maintain care management documentation in accordance with state and federal laws.
Required Qualifications
- Bachelor's degree, preferably in social work, human services, nursing, psychology, or sociology
- Three years working for an Aging Services Access Point organization with case management experience
- at least one year of supervisory experience
- Strong analytical, verbal, and written communication skills
- Excellent interpersonal and collaboration skills
- Experience in team development and staff training
- Proficient in Microsoft Office (Word, Excel) and database systems
- Ability to interpret and apply relevant laws and regulations
- Must be able to work with a diverse population and within a multi-disciplinary team
- Must be familiar with the federal and state programs and local resources
- Must have strong computer skills, including knowledge of Microsoft Office and SAMS
- Understanding of HIPAA regulations and data privacy laws as they relate to client information
Desired Qualifications
