Job Summary
Manage a caseload of 24 families by establishing trusting relationships through regular home visits and utilizing the Parents As Teachers curriculum. Document interventions, observations, and family goal plans within 48 hours of contact, ensuring timely assessments and screenings. Collaborate with the HFM Team Leader to create and revise service plans while maintaining cultural competence with at-risk families. Requires bilingual fluency in English and Spanish, a valid driver's license, and the ability to work flexible evening and weekend hours.
Required Qualifications
- Minimum of high school graduate, or equivalency
- demonstrated knowledge of parenting skills
- early childhood

- Preferred - Bachelor's degree in social work, early childhood, family studies or related field with one year of experience, or experience working with families, infants/children in a home setting
- Strong organizational skills in order to balance the family's needs with the program's requirement for accurate and complete documentation
- Experienced in the use of a computer for documentation, reporting and use of the internet
- Ability to maintain flexible work schedule, including some evening and weekend hours
- Must have a valid driver's license with less than three points and the use of a reliable insured vehicle for work purposes
- Must be able to demonstrate fluency in Spanish and English
