Job Summary
Conduct assessments of individuals and families regarding mental status, behavioral patterns, and family interactions to propose service and treatment plans. Deliver activity and participation-based interventions derived from training and peer successes, while providing 24/7 coverage for assigned cases. Build relationships with referring agencies to guide families and ensure confidential, accurate documentation is submitted to supervisors. Develop closure and aftercare plans for each client, participate in team meetings, and may provide training or field supervision of peers. Travel up to one hour as needed to support families served.
Required Qualifications
- Bachelor's Degree in related Human Service field
- One year of case management or behavioral health experience
- Bi-lingual (Spanish-English)
- Specialized training
- Self-motivated and detail-oriented with ability to multi-task
- Excellent communication and analytical skills
- Valid Driver's license and reliable vehicle, including valid registration & insurance
