Job Summary
Conduct community outreach and education programs to promote home health services to patients, families, and healthcare providers in Brooklyn. Build relationships with community organizations and social agencies to identify partnerships and referrals. Identify and address the social, emotional, and physical needs of patients by providing personalized support and resources. Collaborate with a multidisciplinary team to develop individualized care plans that meet unique patient requirements. Deliver language-specific outreach and education programs to ensure bilingual patients receive necessary care and support. Fluency in English and Spanish is required.
