Job Summary
Community Health Worker (CHW) handles culturally responsive, community-based services to address social drivers of health, including care coordination, system navigation, health education, and advocacy. Works with Medi-Cal CHW requirements to promote stability and improved well-being, engaging participants through strengths-based, trauma-informed practices; duties include intake/eligibility, SDH screenings, action plan development, referrals, documentation in EHR, collaboration with providers, and participation in quality improvement. On-site at Lake Elsinore, CA and field-based with flexible schedule including evenings/weekends; non-exempt, hourly pay $26-$30 depending on experience.
Required Qualifications
- High school diploma or equivalent required
- Minimum of two (2) years of experience in community-based services, care coordination, or related human services work
- Bilingual in English and Spanish (required)
- Strong written and verbal communication skills
- Ability to manage multiple priorities and maintain timely, accurate documentation
- Basic computer proficiency, including experience with electronic records systems
- Valid California driver’s license, reliable transportation, and proof of auto insurance
