Bilingual Community Care Specialist

Job Summary
Conduct ongoing client evaluations, update treatment plans, and manage discharges for vulnerable adults with co-occurring mental health and substance use disorders. Deliver direct and indirect care across mobile platforms, coordinating case management with internal departments and external agencies to improve discharge planning and post-residential engagement. Assist clients by enhancing life skills, securing employment supports, and addressing gaps in care and social determinants of health to increase daily functioning. Meet predetermined monthly billable service goals while attending mandatory staffing, clinical oversight, and ART meetings. Requires bilingual Spanish/English proficiency, current sobriety, and valid driver's license for travel between facilities and community locations.
Required Qualifications
- Bilingual in Spanish and English
- Must currently have a minimum of 6 month of continuous sobriety
- Current CPR/First Aid/Narcan training
- Current and annual negative TB Test
- Current and valid Fingerprint Clearance Card
- Clean Motor Vehicle Record (MVR) check performed annually and upon hire
- Successfully pass a background check and an Arizona Department of Health Services Adult Protective Services check
- Must be able to communication both verbally and in writing
- Ability to work independently and as a member of a team
- Minimum of High School Diploma or GED
- 1 year experience providing direct client care and working in multiple settings related to substance abuse and mental health
- Valid AZ Driver's License
- Current Interpreter Certification or ability to obtain within 90 days of hire
- Days, nights, weekends, and holidays
Desired Qualifications
- Arizona Peer Recovery Support Specialist Certification
- 1 year or more of experience working in a mobile outpatient case management team
- Knowledge of Primary Care Practices
- Clean Motor Vehicle Record (MVR) check performed annually and upon hire
