Job Summary
Clinical Support Nurse – PD Telecare (RN) provides remote patient monitoring and clinical oversight for at-home PD; collaborates with PD program nurses, documents assessments and actions, reviews RPM data daily, escalates clinical concerns, coordinates virtual care, and contributes to Telecare workflow improvements. Must be a bilingual RN with PD/home-dialysis experience, knowledge of RPM/telehealth tools, and strong documentation and analytical skills; ISPD Canadian home-dialysis guidelines knowledge and device troubleshooting are preferred.
Required Qualifications
- Bachelor of Science in Nursing (BScN) or equivalent
- 2–4 years of Telehealth Nursing, Peritoneal Dialysis or Home Dialysis experience
- RN in good standing with a Canadian provincial regulatory body
- Bilingual (English and French)
- Knowledge of APD therapy and cycler troubleshooting
- Experience with Remote Patient Management systems, telehealth platforms, or virtual-care coordination
