Job Summary
Clinical Care Nurse (RN) in a clinic-based role focused on transitions of care, improving Medicare Advantage Stars/HEDIS quality, and coordinating post-discharge education and follow-up. Responsibilities include analyzing clinical data from Athena EMR and DataHub to identify gaps in care, coordinating post-discharge follow-up, conducting outreach via phone, telehealth, and in-clinic visits, educating patients on preventive and chronic disease management, ensuring discharge instructions understood, and collaborating with interdisciplinary teams to optimize workflows and meet CMS regulatory standards. Requires in-center presence, 5 days/week, with travel within the market as needed; bilingual in English and Spanish; strong communication, data interpretation, and motivational interviewing skills.
Required Qualifications
- Associate’s degree in nursing (ADN) or Bachelor’s degree in nursing (BSN). Active, unrestricted RN license (state specific as applicable).
- 3+ years' clinical nursing experience with exposure to transitions of care, quality improvement, managed care, or population health management.
- Proficiency with electronic health records (e.g., Athena EMR), data analytics tools (e.g., DataHub, Compass Rose, SalesForce HealthCloud – per your prior employer’s population health tools), and Microsoft Office Suite.
- Willing and able to complete and maintain Basic Life Support training.
- Bilingual in English and Spanish with full professional proficiency.
