Job Summary
Clinical Care Nurse (RN) in a clinic-based role focused on transitions of care, reducing avoidable ED utilization, and driving Medicare Advantage Stars and quality performance. Responsibilities include analyzing clinical data from platforms such as Athena EMR and DataHub to identify gaps in care related to Stars and HEDIS measures, coordinating post-discharge follow-up for recently discharged inpatients, observation and ED patients, conducting targeted outreach via phone, telehealth, and in-clinic visits to close care opportunities, providing tailored education on preventive care and chronic disease management (including DM, HTN, CHF, COPD), ensuring accurate documentation compliant with CMS regulations and HIPAA, participating in center huddles and quality improvement projects, collaborating with interdisciplinary teams, and supporting clinic operations and community education. Requires in-center presence with occasional market travel; hours are Mon–Fri 8:00 AM–5:00 PM with 5 days/week in clinic; reside in designated market area.
Required Qualifications
- Associate’s degree in nursing (ADN) or Bachelor’s degree in nursing (BSN)
- Active, unrestricted RN license (state specific as applicable)
- 3+ years' clinical nursing experience with exposure to transitions of care, quality improvement, managed care, or population health management
- Proficiency with electronic health records (e.g., Athena EMR)
- Data analytics tools (e.g., DataHub, Compass Rose, Salesforce HealthCloud) – per prior employer’s population health tools
- Microsoft Office Suite
- Bilingual in English and Spanish with full professional proficiency
- Willing and able to complete and maintain Basic Life Support training
