Job Summary
Conduct regular patient outreach for remote patient monitoring and chronic care management programs, including monthly outreach calls. Triage patient needs, review remote monitoring data for abnormal findings, and coordinate with providers and clinical staff. Manage an assigned patient panel by building ongoing relationships, ensuring compliance, and escalating non-compliance issues. Document patient interactions, vitals, and updates in the EMR/EHR system while providing patient education on care programs and resources. Work Monday–Friday, 8:30 AM–5:00 PM, earning $41,600–$49,920 annually with health insurance, PTO, and a monthly productivity bonus.
Required Qualifications
- 2–5 years of experience as a Certified Nursing Assistant (CNA) or in a health customer service role
- strong tenure and experience managing a high call volume
- Bilingual in Spanish or Farsi
Desired Qualifications
- Experience with vital sign readings or a similar clinical support environment
