Bilingual Case Manager - SOT Program

Job Summary
Maintain accurate case records and provide direct case management services for participants in the SOT program. Explain public benefits, assist with completing applications for Medicaid and SNAP, and coordinate referrals to healthcare, employment, and education resources. Offer culturally and linguistically responsive support by facilitating care coordination, arranging transportation, and providing interpretation when needed. Participate in weekly supervision, daily group activities, and internal team meetings to review cases and identify service gaps. Build relationships with community-based organizations and support organizational efforts to strengthen language access policies.
Required Qualifications
- Associate degree
- Full professional fluency in English
- Fluency in at least one of the main languages spoken by participants in the SOT program (French, Spanish, Arabic, and/or Portuguese)
- Native-level proficiency in French, Spanish, Arabic, and/or Portuguese
- Excellent oral and written communication skills
- Demonstrated competency in public speaking
- Strong interpersonal skills
- Proficiency in Microsoft Office
- Proficiency in Internet Explorer
- Proficiency in Outlook/Exchange
- Proficiency in Windows operating systems
- Ability to add, subtract, multiply, and divide into all units of measure, using whole numbers, common fractions, and decimals
- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form
- Ability to problem solve
- Demonstrated commitment to anti-racism principles

Desired Qualifications
- Higher education (Bachelor's Degree or higher)
- Experience attained via a degree higher than a Bachelor's Degree
