Job Summary
Lead Care Manager role primarily remote with 2-3 on-site community visits per month near your city, focusing on outreach and engagement with individuals living with Serious Mental Illness, Substance Use Disorder, homelessness, or high medical needs. Responsibilities include maintaining a caseload, using relationship-based strategies for social support navigation, conducting periodic telephonic/SMS outreach, developing Health Action Plans/Shared Care Plans with members, supporting multidisciplinary team members (nurse care manager, behavioral health care manager, nurse practitioner, Community Engagement Specialist), participating in care reviews, ensuring monthly encounter metrics compliance with health plan regulations, helping individuals secure connections to community supports, coordinating physical care management appointments with external/internal providers, and leveraging online platforms to collaborate with team members. Requirements include 1+ year of general work experience (case management preferred), bilingual English/Spanish, status to be located in California, reliable transportation for on-site engagement, HIPAA-compliant remote workspace, strong cultural fluency, empathy, collaboration, optimization mindset, organizational skills, comfort with technology (CRM, Excel, Word, email, video conferencing), and a valid driver’s license with auto liability insurance. Preferred qualifications add lived experience with complex chronic needs or homelessness/Substance Use Disorder, 2+ years of case management, motivational interviewing, medical terminology knowledge, problem-solving mindset, adaptability, and multitasking ability. Salary range is $22-$25 per hour, with benefits including health insurance, 401k, WFH stipend, and travel reimbursements.
Required Qualifications
- 1+ years of general work experience (Case Management preferred)
- Bilingual – English/Spanish
- Physically located in California
- Reliable transportation for engagement at clinic, community-based organization, and health system partner locations
- Virtual Ops requires a quiet, HIPAA compliant and internet connected space
- Strong understanding of cultural fluency

- Ability to work collaboratively in a multidisciplinary team
