Job Summary
Lead Care Manager responsible for maintaining a caseload of individuals with complex needs (e.g., Serious Mental Illness, Substance Use Disorder, homelessness) using evidence-based engagement strategies; conducts outreach by phone and SMS, develops Health Action Plan/Shared Care Plan with members, and collaborates with nurse care managers, behavioral health care managers, and other professionals. Majority of duties performed remotely from home with 2-3 on-site community visits per month near your city in California; requires strong empathy, cultural fluency, organizational skills, and ability to navigate community resources. Salary range $22-$25/hour equivalent annualized to $45,760-$52,000, plus benefits and work-from-home stipend; full-time, Senior-level Lead Care Manager position with opportunities for growth.
Required Qualifications
- 1+ years of general work experience (Case Management preferred)
- Bilingual – English/Spanish
- Physically located in California
- Valid driver’s license and auto liability insurance
- Ability to work in a multidisciplinary team
- Strong technical skills and comfort with technology (CRM databases, basic Excel, Word, email, video conferencing)
- HIPAA compliant remote work space or quiet area for Virtual Ops
