Job Summary
Coordinate case management services for children in care, conducting on-site admission interviews, sponsor vetting, and weekly updates to youth and families. Maintain active caseloads of 8 cases while ensuring timely assessments, referrals to external providers, and continuity of health and legal services. Drive children to court appointments and reunification services, manage UC Portal and physical file compliance, and report significant incidents per policy. Work flexible on-call schedules including weekends and evenings to ensure safe, timely release from care.
Required Qualifications
- Bachelor's degree in the behavioral sciences, human services or social services fields
- Fluency in Spanish
- Minimum automobile insurance coverage of $100,000/300,000 bodily liability coverage
- 21 years of age
- Valid state driver's license
- Reliable personal vehicle
- Good driving record
- Ability to work a flexible schedule to facilitate program services, including working on call schedules which includes weekends and evenings
- Must be able to work 8 UC cases with minimal supervision
- Ensure the proper supervision of youth at all times
- Demonstrates tolerance and respect for the ideas and actions of others
- Possess a sincere desire and ability to advocate for children facing behavioral health, immigration, educational, legal, development, and socio-economic challenges
- Use of manual dexterity, tactile, visual, and audio acuity
- Use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands
- Occasional lifting (up to 25 pounds), bending, pulling, and carrying
- Ability to travel frequently and drive vehicle while sitting for extended periods
