Job Summary
Full-time Bilingual Case Manager for the Healthy Families Suffolk program. The Case Manager conducts home visits to assess family needs and determine program eligibility, performs interviews and completes assessments with expectant parents or parents of a newborn, and provides home-based prevention services focused on child development education and coordinating services for families. Responsibilities include documenting assessments and visits, coordinating with supervisor on referrals and cas assignments, maintaining required records, conducting community outreach to sustain referrals, advocating for participants, providing short-term direct assistance with supervisor approval, collaborating with community partners, and participating in team activities and CQI initiatives. Flexible scheduling including limited nights and weekends; must be fluent in Spanish and capable of building trusting relationships with mothers, fathers, and extended family.
Required Qualifications
- High school diploma or equivalent required
- NYS-AIMH Infant Mental Health Endorsement is a plus
- At least 6 months' experience in working with or providing services to children and families required
- Knowledge of infant and child development required
- Proficient computer skills, including Microsoft office, required
- Excellent verbal and written communication skills required
- An ability to establish trusting relationships and a willingness to engage in building reflective capacity
- Valid and clean NYS Driver’s License required
- Bilingual in Spanish required
