Job Summary
Lead Care Manager responsible for directly outreaching and engaging individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those with high medical needs. Maintain an ongoing caseload, use relationship-based strategies to support members, conduct periodic telephonic and SMS outreach, identify health/wellness goals and incorporate them into a Health Action Plan/Shared Care Plan, and collaborate with nurse care managers, behavioral health care managers, nurse practitioners, and the Community Engagement Specialist. Ensure monthly encounter metrics compliance, assist individuals in connecting to community supports, coordinate physical care management appointments, and leverage internal/external platforms to collaborate with team members. Requires 1+ years of general work experience, bilingual English/Spanish, California residency, reliable transportation for on-site engagement, HIPAA-compliant remote workspace, and a strong blend of empathy, organizational, and technical skills. Salary is $22-$25/hour with remote-friendly structure (primarily work-from-home with 2-3 on-site visits per month near your city). Location noted as Long Beach, CA.
Required Qualifications
- 1+ years of general work experience (Case Management preferred)
- Bilingual – English/Spanish
