Job Summary
Coordinate care plans and assess member needs using data analysis tools to identify risks and facilitate access to healthcare services. Collaborate with internal teams, providers, and external resources to address barriers, improve quality of care, and ensure services align with member benefits. Promote member engagement through communication, coaching, and motivational techniques while monitoring care plan progress and documenting activities for compliance. This full-time role in Hialeah & Hialeah Gardens requires 75% travel and bilingual proficiency in English and Spanish. Applicants must reside in zip codes 33012 & 33010 and hold a Bachelor's degree or Master's in behavioral health or human services.
Required Qualifications
- Bilingual (English & Spanish)
- Must successfully pass both verbal and written language assessments
- Must have reliable transportation
- Must be willing/able to travel up to 75% of the time
- Must reside in Hialeah or Hialeah Gardens, FL (within zip codes 33012 & 33010)
- At least 1 year in a behavioral health or long-term care setting
- Proficiency in Microsoft Office Suite
- Proficiency in computer use
- Bachelor's Degree
- non-licensed Master's level clinician
- Degree in behavioral health, human services (psychology, social work, marriage and family counseling)
Desired Qualifications
