Job Summary
The Bilingual Care Manager RN at Monogram Health provides critical care management for patients facing multiple chronic conditions, emphasizing quality of life improvements at home. Responsibilities include developing and adapting care plans, performing in-home care management visits, monitoring biometric data, coordinating with medical providers, and educating patients on chronic kidney disease (CKD) and other health conditions. Candidates must have at least 2 years of relevant experience, a Bachelors's degree in nursing, and a valid RN license. Bilingual proficiency in English and Spanish is required.
Required Qualifications
- Bilingual (English/Spanish)
- Currently licensed as a Registered Nurse in the State of the posted location
- Graduate of an accredited School of Nursing
- Basic Life Support (BLS) certification required
- 2+ years previous experience working in care management and/or with CKD/ESRD patients
- Excellent verbal communication skills both in person and on the phone
- Self-starter with the ability to work independently with minimal supervision
