Job Summary
Registered Nurse in-home care management for CKD/ESRD patients; develop and adapt care plans with providers; perform in-home visits, monitor vitals and administer/coordinate medications; educate patients and families on CKD, dialysis, and related lifestyle; coordinate with dialysis providers; travel to Sierra Vista, AZ for two weeks per month (mileage, meals, and hotel covered); must reside in Arizona; BLS certified; 2+ years in care management; Spanish/English bilingual; full benefits and relocation bonus.
Required Qualifications
- Graduate of an accredited School of Nursing
- Current Registered Nurse license in the State of Arizona
- BLS certification
- 2+ years experience in care management and/or CKD/ESRD patient care
- Must be bilingual English/Spanish
- Graduate of an accredited School of Nursing (RN)
- Ability to travel locally to perform in-home visits
