Job Summary
Bilingual Care Coordinator for the Children’s Long-Term Support (CLTS) Waiver Program in Milwaukee County. Responsibilities include assessment, service planning, and implementation of individualized care plans for children with developmental, physical, or emotional disabilities, using trauma-informed and family-centered approaches. Must be fluent in Spanish and English; position supports hybrid remote work and is full-time, Monday–Friday, first shift. Includes documentation, time reporting, coordinating services, and collaboration with clients, families, and multidisciplinary teams; travel may be required.
