Job Summary
Conduct home and telephone assessments to link clients with primary care, dental, specialty, and preventive services. Educate residents on Maryland Medical Program benefits and assist with barriers to healthcare including housing, addiction treatment, and energy assistance. Manage a caseload of approximately 20 clients, closing Pre-natal Risk Assessment referrals within 10 days and other local health service requests within 30 days. Provide culturally competent outreach to the immigrant and migrant populations in Baltimore City. Submit accurate timesheets and complete required trainings while adhering to Standard Operating Procedures.
Required Qualifications
- High School Diploma or GED
- Significant human services experience
- 2 years of experience working in an administrative support capacity with a multidisciplinary team
- Ability to speak, read and write in Spanish
- Reliable transportation
- Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365)
- Organizational skills with the ability to meet a demanding workload
- Detail oriented to complete requirements of files and contract compliance
- Sensitivity to cultural and socioeconomic characteristics of population served
- Excellent oral and written interpersonal and communication skills required
- Problem solving skills
- Proven ability to work efficiently and meet deadlines
- Must regularly lift and/or move up to 25 pounds
- Must occasionally lift and/or move up to 50 pounds

Desired Qualifications
- Bachelor's degree in a Human Services field
- Experience working within substance abuse recovery population
- Community Health Worker Certification
- Knowledge of the Housing First philosophy
