Bilingual - Bachelor Level Mental Health Counselor - Integrated Care

Bachelor-level Mental Health Counselor in the Integrated Care program for the Public Health Mobile Unit serving Weld County. Conducts initial and ongoing assessments to identify mental health needs, provides crisis intervention, short-term counseling, individual/family/group therapy, and delivers case management to support client stability and continuity of care. Develops strength-based service plans, monitors treatment progress, maintains accurate electronic clinical records, and coordinates care with internal teams and external partners, collaborating with a clinical supervisor, multidisciplinary team, and psychiatrist to ensure coordinated, culturally responsive care. Requires bilingual English/Spanish and supports a range of services including vaccinations, STI testing, maternal/child health support, and health education within the Mobile Unit.
